Enrollment Requirements:

All students must be completely toilet trained.
Students must be 3 years of age or older.
Young 5s students must be 4 years 6 months or older by July 1, 2024.
A Health Appraisal is due 30 days from the student’s first day of school. All other forms must be turned in by the first day of school. Health Appraisals are valid for 2 years from the date signed by your physician.
Required immunizations and/or complete waivers MUST be completed and on file in the office by 08/22/2024 to allow for required reporting to be submitted. *If not on file by 08/22/2024 your child can not attend class until on file per State Law. FERPA or waiver must be turned in with enrollment.
Written Information Packet Documentation form signed stating that you are aware of the Early Childhood
handbook location on the website and can access the information. Hard copies of the handbook are available by request.
In the event that a child is enrolled and not ready to attend either due to age, potty training
issues, or other reasons, the parent has the right to hold the child’s place in the class by
paying the tuition as stated on the contract until the child is ready to attend. If tuition is not
paid, the space will be made available to the next interested student.

Complete the Early Childhood New Student Enrollment form
Required forms by the State of Michigan:
- Child Information Record
- Written Information Packet One per family is required.
- Health Appraisal (expires after 2 years)
- Copy of Birth Certificate
- Immunization Records
Required forms by Faith Lutheran School:
- Enrolling parent's driver's license or state ID
- Proof of custody (any and all court documents pertaining to your child's custody)
- A copy of IEP/NPSP/ETR from the previous school (helpful)
